When you made a donation or purchased merchandise from the campaign you did not provide all of the necessary information required by the Federal Elections Commission (FEC). We need to report the name, address, occupation and employer of individuals who contribute to our campaign. 

You are receiving this message because you provided invalid information when you donated and we need to correct this information or we will be fined by the FEC.

The Federal Election Campaign Act requires the following information for contributions:

  • Full Name
  • Mailing address
  • Occupation and Employer 
  • US Citizen or "green card" holder

A few things to remember when filling out the form:

  • If you don't have an employer, put "Unemployed" , "Student" or "Retired"

  • If you are self-employed put "Self-employed" in employer and describe your occupation. "

  • N/A" is not an acceptable entry.

  • If you are donating from outside the United States, you need to confirm that you are a U.S. Citizen by checking the box below.
